If you have any difficulties or you would prefer a paper copy of this form in regular or large print, or any other format please telephone the Ombudsman's Office on 0845 601 0794, the staff there will try to help.

If you complained about more than one lawyer you should complete separate online applications for each one.


You don't need to repeat the complaints about the lawyer.

The Ombudsman will have access to the professional body's file and this will contain all the correspondence that you sent to them.
